Arkansas cab card have been enlarged to print 8 1/2 x 11 light blue paper. The cab card issued to each registrant will have an "A" in the lower right hand-corner of the cab card. The upper right- hand corner contains a bar code.
The expiration date, decal, and plate number are found just above the registrants, owner, and vehicle information. The jurisdictions and weights for which the registrant is apportioned with are found on the bottom half.

Description

The California Apportioned Cab Card, form number REG. 499, is printed on one half of a regular 8 ½” by 11 ½” plain white paper. The area where all information is printed is colored in a very light lime green. The State of California, Department of Motor Vehicles logo is printed on the top left hand side of the card. The seal of the state is printed on the background of the carrier’s name towards the low center. The dimension of the cab card is 8 ½” X 5 ½”.
The information is divided into 2 sections: A square area measuring 5 ½” x 5 ½” inches, printed with the following information, from left to right and top to bottom: Apportioned registration expiration date License plate number Date of registration Supplement type/number Vehicle make Vehicle year model Account number Vehicle’s number in the fleet (equipment number) Vehicle Identification Number (VIN) Type of vehicle Unladen weight Number of axles ( Quaxles for Quebec) Purchase price Purchase date Type of carrier, such as private or for-hire Number of seats (for busses) Motive power, such as gas or diesel Registrant/Lessee’s name and address Owner/Lessor’s name and address
The remaining area of 3 inches wide by approximately 4 ½ inches high is delineated and is used to list the names of the jurisdictions the carrier has applied for and the maximum vehicle gross weight allowed in each of these jurisdictions.
Apportioned Registration Renewal is done on a calendar yearly basis. Original registrations may be submitted at any time during the year. Following is a copy of the California Apportioned Cab Card. |

Colorado

Colorado’s Cab Cards are printed on 8 ½ by 11 plain white paper stock without the State of Colorado Department of Revenue seal.
The Cab cards are printed on (2) computer systems:
1) "CVEC" Commercial Vehicle Electronic Credentialing Program. This cab card will have in the upper left hand corner "CVEC IRP Cab card" and in the upper right hand corner "Page _ of _". On the bottom left hand side of the cab card it will show the web site that the cab card was printed from and in the lower right hand corner a date of when the cab-card was printed.
2) “In House” program of Motor Carrier Services, Service Section. This cab card has copy of (2) Cab cards, one upright and the other upside down that has a perforation that can be torn off . |

Connecticut
Description
|
Connecticut’s Cab Card is produced on an 8 ½”x 11” special form. Each form is produced with a unique document control number that is printed in red ink in the upper right-hand corner of the form. The top one-third of the form is shaded in green ink and contains the owner, vehicle and registrant information The system generates a unique cab card number that is printed in the green-shaded area to the right of the owner’s name. The middle one-third of the form contains the mailing address of the registrant and a PDF-417 bar code. The bar code is used by law enforcement authorities in their efforts to promote safe vehicle operations on the roadways. |

Idaho

The Idaho apportioned cab card is 4” x 9” on card stock, The background is pale blue with the Idaho state seal in white, located on the right-hand side behind the apportioned jurisdictions and weights. The cab cards are printed on LaserJet printers with all alpha/numeric characters in black. The cab card will reflect the required information for the registrant, owner and the vehicle in the appropriate spaces. The registrant must sign the cab card in the space provided. The reverse side of the cab card is white with all alpha/numeric characters printed in black. The Idaho Transportation Department logo is located in the upper left-hand corner. Information such as Instructions for canceling the registration and Motor Carrier Safety and Insurance requirements is located on this side. During business hours (Monday through Friday, 8:30 am. to 5 p.m. Mountain Time), cab card validity can be confirmed by contacting the Idaho Transportation Department, Motor Carrier One Stop Shop at (208) 334-8611. After hours, a copy of the cab card in question can be faxed to (208) 334-2006 along with a return telephone number. Someone will contact that number as soon as possible to determine legitimacy. |

Indiana's cab cards are printed on 81/2 by 11" plain white or color paper, and there are no water marks or special markings on the paper. The Indiana State seals are located at the top right and left hand corner. Depending on the printer used, the state seals may be or may not be in color. Whether the seals are in color or not in color does not affect the validity of the cab card.
The enforcement control number listed before the jurisdictions is unique to each vehicle. The validity of the cab card/vehicle can be verified through the Indiana's Voice Response Unit 1-866-615-7340 available 24 hours each day.

We have reduced the size of the layout of our trailer cab cards. The previous layout covered a full 8.5x11 standard stock paper. The new version will be printed on 8.5x11 standard stock paper however the layout will cover slightly less than half the sheet. The new version of the trailer cab card will be issued to any trailer upon initial registration, a replacement plate or replacement cab card transaction and the renewal. Previously issued full page trailer cab cards are still valid until expiration.
Carriers are permitted to trim the excess paper from the trailer cab card to fit their cab card holders affixed to the trailer. This does not affect the validity or “original” status of the cab card. Also, the state seals may or may not have color depending on the printer used by the carrier when printing the trailer cab cards.

Kansas
Description
|
Until March 2, 2009, Kansas will have three types of valid cab cards for power units and trailers. Listed below are valid card descriptions:
- Cab cards issued from the IRP office are printed on 8 ½ X 11 piece of plain white paper with a background of the State of Kansas, Department of Revenue seal.
- A cab card printed via the Internet. It has a small Kansas Department of Revenue seal in the upper left corner of the cab card. It is printed on 8 ½ X 11 paper also.
- The new cab card, effective June 9, will have the State of Kansas, Department of Revenue seal in the upper left corner and will have either one or two bar codes in the lower portion. The bar codes meet PRISM specifications. This card is printed on plain white 8 ½ X 11 paper.
With the 2009 renewal season Kansas will reprint all cab cards (power & trailer units) and after March 2, 2009 the only valid cab card will be number three listed above. |

Massachusetts
Description |
The Commonwealth of Massachusetts is currently in the process of converting to laser generated credentials. Because of this change, the cab cards and IRP Temporary Vehicle Registrations issued to Massachusetts-based IRP Vehicles will now be printed on 8 ½” x 11” paper with an electronic signature of the Registrar of Motor Vehicles, Anne L. Collins. Please see the attached copies of the new versions of the Massachusetts IRP Cab Card and Temporary Registrations. Beginning with the 2009 renewal cycle, all cab cards will be printed in the new format. Until then, IRP jurisdictions and law enforcement agencies will see the current and new versions of MA cab cards and Temporary Registrations.

Mississippi’s Apportioned Cab Card is generated from a Dot Matrix Printer on continuously fed paper. The cab card is a pale yellow form with perforated edges. The size of the cab card is an 8” X 4”.
The information on the cab card from left to right is as follows: First Row: License Number (Power Unit has a 7 digit field, Permanent Trailer has a 6 digit field), Account Number (5 or 6 digit field), Fleet Number Second Row: Unit Number, Year, Make, and Vehicle I.D. (VIN) Number of Vehicle Third Row: Empty Weight, Gross Weight, Title Number Forth Row: County (N/A), Decal (Power Unit has a 8 digit number, Permanent Trailer has the word Perm), Type of Carrier (HH or PC) Fifth Row: Registrant Sixth Row: Owner of Vehicle Seventh Row: Street Address Eight Row: City, State, and Zip Code Ninth Row: Date Registered, Date Expires (Required only on Power Units), Grace Period Expires (Required only on Power Units) Tenth Row: This card must be carried in vehicle.
There should be NO eraser marks, whiteout, nor hand written corrections on the cab card.
On the right side of Mississippi’s Apportioned Power Unit Cab Card is the jurisdiction(s) the registrant is registered to travel with the gross combined weight. Power unit cab card account number reflects the expiration month. Example: Account number is 111001 and the expiration is the last day of November.
Apportioned Permanent Trailer Cab Card will only have MS with Qual followed by asterisks in the weight field. Permanent trailer cab card has no expiration date. |

New Brunswick

The New Brunswick IRP document serves both as a registration (ownership document) as well as a Cab Card to authorize travel in IRP jurisdictions. It is printed in both Canadian Official Languages (English and French). The document is approximately 8 ½ inches x 5 ½ inches in dimension. It is printed on 40 weight paper with a white background. There are two colors used by the manufacturer in pre-printing the document, which are green and maroon. The green is used primarily for titles of data element fields and instructions while the maroon fields are used for the form and department name and the shading of key data element boxes (i.e. plate number, effective date, expiry date, National Safety Code number, and signature area.).
The form is printed in two copies which are labelled as being: Registered owner copy, and Truck copy to be kept in vehicle
The “registered owner copy” has a transfer capability printed on the reverse side where the owner could “sign-off’ the ownership of the vehicle in the event it was being sold. There is no reverse side printing on the “truck copy”. Both forms are valid. NB issued two forms so that a carrier could retain one copy and the trucker would have an original for use in the vehicle. The form has the ability to show the broker name and address or both the broker name and address as well as the carrier’s name and address. All forms are pre-numbered by the manufacturer with an inventory control number that is printed in the maroon color and has the format “CC” followed by a variable six digit number, e.g.CC123456. There are no special security features built into the current form. There is an internal form number in the bottom right copy of each form which indicates the number assigned to this form by forms management as well as the month and year it was last revised, e.g. 78-9303 (1/02) is on the current form. |

New Hampshire is in the process of implementing a new IRP cab card. The cab cards will be laser printed on an 8 1/2 x 11 piece of white paper with the New Hampshire State Seal in the center as a background. The cab card will contain a bar code in the lower right hand corner and have a smaller version of the State’s Seal inscripted on the lower left-hand corner of the registration. All information on the cab card will remain unchanged with the exception of two new fields consisting of the Vehicle US DOT number and the Vehicle TIN. |

The upper left quarter of the New York’s Cab Card is a sticker for the windshield of the registered vehicle. The sticker should be placed in the lower driver side portion of the windshield. The upper right quarter of the document is the ownership document for vehicles that are not titled in New York (1972 and older vehicles). The lower half of the document is the cab card. As you can see, these documents are numbered and the number on the cab card should match the number on the windshield sticker. |

Prince Edward Island

Prince Edward island’s cab cards are laser printed on an 8” x 11” piece of paper with the provincial wordmark in the upper left hand corner. As an additional security feature, a tractor and trailer combination watermark with the words “Prince Edward island” is printed at time of issuance in the upper right hand corner. A cab card number is also assigned at time of printing.
New Cab Card Description Effective Currently: PEI is a staggered jurisdiction, the old version (Listed above) will be in circulation until January 2007.
Prince Edward Island's new cab card is a laser printed in black ink, on an 8 1/2 by 11 inch piece of white paper with a full colour Prince Edward Island coat of arms in the upper left hand corner. As an additional security feature, the cab card will have an "embossed" coat of arms in the lower right hand corner of the document. A cab card number will also be assigned at time of issuance.

Quebec
Description

Quebec’s cab card is laser printed on 8 ½” X 11” secured paper with the Societe de l’assurance automobile du Quebec letterhead. Information on the account number, identification and fleet numbers, effective and expiry dates are written on the right side, next to the green highlighted squares. The complete name and address of the registrant is on the upper left side. Green is also the background color of the larger part used to write down the name of the jurisdiction and the weight requested and the umber of axles for Quebec. Above this section is the carrier’s name and address, owner-operator’s name if the case is, the compete description of the vehicle, VIN, make, model, year, unladen weight, fuel, axles of the power unit, combined axles, and information on the number of seats on a bus. The bottom part is detachable but must remain attached as long as the vehicle is not sold to someone else. IT has the same green background and must be signed by the owner or his representative. The original cab card must be on board the vehicle at all times, and a photocopied cab card or one printed on plain paper is not valid. |

Rhode Island

Rhode Island’s Apportioned Cab Cards are laser printed on an 8 ½” X 11” piece of paper. The Cab Cards are printed on special White Frost Imaging paper. The Expiration Date appears in the top left band corner and the Plate Type/Plate# appears on the top right hand corner of the paper. The “Gross Weight” is listed on the left hand side between axles and fuel. If carrier has an Overweight Permit, that weight will be displayed in the “Gross Weight” box, which will differ from R weight listed on the right hand side. R legal weight is displayed to the right and the max weight, if different (with O.W permit) will be displayed in the “Gross Weight” box to the left. All Cab Cards are stamped in the bottom right hand corner of the paper with Rhode Island’s State Official stamp with Approval/Date that registration was issued. Cab Cards are not valid without RI’s Official stamp. |

Saskatchewan

The top left section of Saskatchewan’s Cab Card is the Company Name and Address. The top right section shows: the Supplement Number, (which consists of the Carrier Account Number, Fleet Number and Supplement Number); the Carrier Type (Private, For Hire or Daily Rental); the NSC #, Vehicle Unit #, Serial Number, Tare Weight, Effective Date and Expiry Date. The middle section is the vehicle registration portion of the cab card containing all the vehicle information, i.e. Year, Make/Model, Serial Number, etc. On the back of the cab card in this middle section is the pink Liability Insurance Card. The bottom section lists all the jurisdictions that the Carder has registered to travel in or through. Note: The jurisdiction of Quebec registers by Axles. Note: The note on the jurisdiction section states: “Cab Cards that are copied, altered, or list jurisdictions after the row of asterisks in Canadian or American groups are invalid”. |

Vermont’s cab card is 8 ½ x 3 ¾ and is green in color, with thatches in the background. The cab card is perforated in the middle, which allows for easy folding. The upper left hand corner of the cab card has the Department’s name and address; the upper right hand corner indicates when the registration expires. The expiration date will be written as Month/Year (DEC 03).
The following information is indicated on the left side of the cab card in the order listed:

Underneath the above information is the Registrant’s (Operator/Lessee) Name and Address. On the left side of the cab card, under the expiration date, the jurisdictions in which the vehicle is proportionally registered, and at what weight or axle limits, are listed. Rows of asterisks will fill in remaining blanks once all authorized jurisdictions have been listed. Any jurisdictions listed below the asterisks are invalid. Authority is granted under 23 V.S.A Sec 305(a). There is a small rectangular box underneath the jurisdictions and asterisks; below the box it reads Receipt Line Is Your Record Of Fee Paid and Validates This Cab Card. The validation for the cab card is a round black ink stamp, which has Vermont Department of Motor Vehicles, the date of cab card issuance and the Commissioners Signature inside the stamp. Notes on the left hand side of the cab card indicate that the Card must be carried in the vehicle to which it is issued or be subject to confiscation. Copies Are Not Valid. The back side of Cad Card is Blank.
New Card Description: The cab cards will be laser printed on an 8 ½ X 11 piece of light green paper with the Vermont State Seal in the background. The cab card will also contain a bar code on the bottom right side of the credential. To the left of the bar code is where the validation stickers will print; these must be attached to the license plates. The information on the cab card has remained the same with the exception of two fields, the Vehicle US DOT and Vehicle TIN. |

The WA IRP cab card is 8 x 11 1/2 inches and is printed on plain white paper using a laser jet printer. The seal of the state of Washington appears in the background. The print is in black ink. The Washington seal in the background is a light gray. The logo of the Washington State Department of Licensing is in the upper left hand portion of the cab card and is a darker black then the printed information on the card. The cab card title says "Washington Registration Cab Card" and is also a darker black then the printed information on the card. The form number is PR-450-131. WA announced on March 15, 2005 that it will begin to issue a new apportioned plate series that will consist of five numbers followed by the letters RP, for example, "00000RP" |

West Virginia’s apportioned cab card is 3 5/8” X 7 ½ ” in size and includes the date registered, expiration date, year and make of the vehicle, vehicle identification number, plate number assigned to the vehicle, owner name, equipment number, carrier name and address, account number, and each jurisdiction and weight/axle in which the carrier is apportioned. Other information provided on the cab card are the carrier type, fuel type, title number, unladen weigh, % mileage factor, and vehicle price. The cab card should also display larger red control numbers in the upper left corner. ======================================== Effective July 1, 2005: The new cab card will be laser printed on an 8 1/2 x 11 inch piece of white paper with a background of the West Virginia State Seal. The cab card will contain a bar code in the lower right hand corner of the document. |

Wisconsin

Wisconsin's cab cards are printed on special stock, which is 4 1/4 x 7 1/2 . The Wisconsin Dept of Transportation logo is on the bottom right side of the cab card. The cab card contains registrant and vehicle information. The jurisdictions for which the registrant's vehicle is apportioned and the corresponding weights are reflected on the front of the cab card.
The IRP temporary vehicle registration produced in house are on 8 1/2 x 11 plain paper. At the bottom right side of the temporary it will contain "Processor ID" which identifies the staff person who issued the temporary. Our ID's start with DOT or DIT. For example you could see and ID of DOTSAS or DITTJG. On the bottom left side of the temp you will see the "Time Issued" which is Central Standard Time.
The IRP temporary vehicle registration produced from the Internet by customers or their authorized representative is on 8 1/2 x 11 plain paper. At the bottom right side of the temporary it will contain "Processor ID" which appears as "Web Credential". All temporaries issued from the Internet will show the ID as "Web Credential". On the bottom left side of the temporary you will see the "Time Issued" which is Eastern Standard Time. The time is different because the transaction goes through our vendors, R. L. Polk server. |
